Request a Fit Note / Sick Notes
If you’ve been off work sick for several days, then you may need to show your employer a fit note (also known as an NHS sick note) from your GP to confirm that you’ve been ill and that you’re able to return to work. You can request a GP fit note using our online system. Find out more below.
How to get a fit note
You can request a fit note by clicking the button below. Once you've submitted your request, a clinician will review it and either issue a fit note to confirm you’re fit to work or arrange an appointment to see you in person. This will depend on your condition and whether you've recently been in hospital.

When to get a fit note
Seven Days or Less
- If you’re off sick for seven days or less, your employer shouldn’t require a fit note.
- They may ask you to confirm your illness and that you’re fit to return to work. This is called self-certification, and you can get the form directly from your employer.
More Than Seven Days
- If you’re off work for more than seven days, your employer may ask for a fit note as evidence from a GP that you are not fit to work.
