Badger Notes is a secure NHS service for managing maternity referrals in Bristol, North Somerset, and South Gloucestershire (BNSSG). This service connects you to the right care from the moment you know you're pregnant.

How It Works

  • Complete a self-referral form online.
  • Your referral is reviewed, and your chosen hospital will contact you to begin antenatal care.
  • Use the service to manage and access important information about your pregnancy.

Before You Start

  • Update your GP with any changes to your name or address before using this service.
  • You will need your NHS Number to complete the referral.

Who Can Use Badger Notes?

  • Pregnant individuals planning care at:
    • North Bristol NHS Trust (Southmead or Cossham).
    • University Hospitals Bristol and Weston NHS Foundation Trust (St Michael's or Weston General).
  • Those referring on behalf of a pregnant person.

How to Refer

  • Click here to complete your referral. The form takes about 10 minutes.
